Office Policies and Insurance

First Appointments

The first meeting with a therapist or counselor is quite similar to a first-time appointment to most other healthcare professionals.  It is essentially, an information-gathering meeting.  There are forms to complete, and policies to review and agreements to be made.  We then meet to gather clinical information, answer your questions, and start forming a plan for how best to meet your needs.

Please bring your insurance card and a method of payment (cash, check, credit card accepted) with you to your first appointment.  Any copays or fees will be due at time of service.

Rates and Insurance

Vitality Clinical Services works with most major insurance plans in order to provide the best and most affordable coverage for our clients.  As such, when receiving services through insurance, expenses will vary depending on insurance benefits.  We will work to check benefits prior to the 1st appointment so that copays, coinsurance, and deductibles are understood ahead of time.  Health Savings Accounts typically can be used to cover psychotherapy.

When paying out of pocket, charges will ultimately vary depending on service rendered.  Clinical services are billed at an out-of-pocket rate of $150 per hour.  Non-clinical services are billed at a rate of $100 per hour.

Insurance

We currently accept the following insurance plans:

BCBS PPO

  • Northwestern Medicine Physician Partners (BCBS HMO)

  • Cigna

  • Humana

  • Optum (United)

  • Magellan

  • Beacon

  • BDA Morneau Shepell

  • American Behavioral Health

Office Policies

All payments are due at the time of service.  A $25 insufficient funds charge will be applied for returned checks.  A late cancellation fee of $100 will be applied for cancellations or rescheduling inside of 24 hours before scheduled appointments (except in cases of disaster or emergency).  Insurance does not reimburse for missed appointments.

In certain cases, insurance may not cover all clinically appropriate services.  In the event that insurance does not cover or denies payment, the recipient of services (or designated party) will be responsible for payment.  It is advised that you check with your insurance company to verify copayments and deductibles.
